A referee in the KHL committed a costly mistake after allowing a goal to stand that shouldn't have, and he's been banned for life because of it.

A costly mistake by a referee in the KHL has resulted in a lifetime ban thanks to his actions doing a recent game between SKA and Avtomobilist.
Defenseman Andrey Pedan was credited with the GWG for SKA, though replays showed that his shot entered the net through the side netting, which would make it not a legitimate goal.
The KHL released the following statement, translated from Russian, explaining that the official in question is now banned from working any further games.
In the 46th minute of the game, defenseman Andrei Pedan's shot failed to cross the goal line. The ice rink video review official, according to procedure, was required to verify the goal during the 60-second pause and only then extinguish the orange light. This official has been permanently banned from officiating KHL games.

The NHL Banned Controversial Referee Tim Peel in 2021 After Hot Mic Incident

The NHL experienced something similar recently, as longtime referee Tim Peel was banned from working any further games after an incident in 2021 between the Nashville Predators and Detroit Red Wings.
Peel was caught on a hot mic saying that he intended to give a penalty right away to the Predators.
It wasn't much, but I wanted to get a f---ing penalty against Nashville early. - Tim Peel

Peel had worked 1,334 games coming into that season, which was his last in the NHL.
